YUMC에서 한국병원 체험, 잊지 못할 추억

- 굿모닝 YUMC... 미국 플로리다에서 온 반가운 편지 -

Not many of my colleagues have the opportunity to observe how medicine is practiced abroad. When I first told my classmates at the University of South Florida (USF) College of Medicine that I would be traveling to Daegu for an observatory rotation, many of them thought it was interesting and were excited for the experience I would have during those two and a half weeks. I did not know what to expect and was a bit apprehensive about the whole ordeal at first. But with support from Dr. John Sinnott at USF and careful support and planning from Dr. Sung Eon Gi at Yeungnam University, I arrived in Daegu early August 2009 to embark on an experience I will never forget.

외국병원을 체험할 수 있는 기회는 흔치 않습니다. 대구로 임상실습을 하러 간다고 했을 때, USF(미국 사우스플로리다大) 의과대학 동기들은 제가 2주 이상 경험할 내용이 무엇인지 관심을 갖고 부러워했습니다. 처음에는 아무것도 몰랐고 걱정도 많이 했지만, 존 시놋(John Sinnott) USF 교수님과 성언기 영남대 교수님께서 성심성의껏 지원해주셨고, 일정을 잡아주셔서 잊지 못할 경험을 하게 됐습니다.

I was fortunate enough to have homestay arrangements with current third year medical student Kwon Jungim and her parents during my time in Daegu. I truly felt as though they were my family away from home. Mrs. Kwon prepared a wonderful breakfast for us every morning, Mr. Kwon drove me to the hospital on numerous occasions, and Jungim helped me adjust to Korea -- giving me advice on the ins and outs of the hospital and sharing about life as a young adult in Daegu. I had a wonderful time with them cooking meat on the roof of their apartment, playing go-stop under the stars, and traveling to Gyeongju to eat soondooboo and sightsee. The whole family took care of me as their own daughter and sister and I could not have asked for more.

운 좋게도 대구에 머무르는 동안 권정임 학생(의학과 3년)과 그 가족 집에서 홈스테이를 할 수 있었습니다. 모두 정말 친가족처럼 느껴지는 분들이었습니다. 어머님은 매일 아침 맛있는 아침식사를 준비해주셨고, 아버님은 수차례 병원까지 저를 태워다주셨습니다. 정임이는 병원사정을 잘 알려주었을 뿐만 아니라 한국 대학생의 삶을 함께 체험하면서 제가 수월하게 적응할 수 있도록 도왔습니다. 그분들과 아파트 테라스에서 고기도 구워먹고, 별 가득한 밤하늘 아래에서 고스톱도 치고, 경주에 가서 순두부찌개도 먹고, 관광도 하면서 즐거운 시간을 보냈습니다. 모든 식구들이 친자식, 친자매처럼 저를 돌봐주셨습니다.

It is always educational to see how things are done at other institutions ; it gives us a sense of what we are doing well and where our own institutions can improve. I rotated through six different departments between August 3rd and August 18th, learning how each department functioned and seeing how medical students, nurses, interns, residents and attending physicians participated on the medical team.

다른 학교에서 일이 어떻게 진행되는지 보는 것은 모교가 무엇을 더 잘 하고 있고, 또 무엇을 더 향상시켜야 할지를 깨닫게 합니다. 저는 지난 8월 3일부터 18일 사이에 영남대학교병원 6개 과에서 실습을 받으며 각각의 과가 어떻게 돌아가는지, 학생▪간호사▪인턴▪레지던트▪전임의가 어떻게 의료 팀에 참여하는지 배웠습니다.

As a medical student myself, I was perhaps most to the differences in the medical education system. In the United States, students typically spend twelve months during their third year of medical school rotating through the major specialties(general internal medicine, pediatrics, general surgery, emergency medicine, obstetrics & gynecology, family medicine, neurology, psychiatry). During our fourth year, we choose eleven specialties(major or minor) and spend one month rotating through each of them. Thus, we have less of an opportunity to explore all the specialties than students in Korea. Another notable difference was in what students are allowed to do as part of the medical team. At USF, on a general internal medicine rotation, students will typically carry six patients of their own at any given time – examining each of them, writing progress notes in their charts, ordering laboratories, medications, and consults every day. I believe the difference in culture plays a large role in this discrepancy.

저도 의대생이기에, 아마도 가장 주의 깊게 보았던 건 의학교육 체계의 차이가 아닐까 합니다. 미국 의대생들은 의학과 3학년 12개월 동안 메이저 과(내과, 소아청소년과, 외과, 응급의학과, 산부인과, 가정의학과, 신경과, 정신과)에서 임상실습을 받고, 4학년 때는 11개 과(메이저 / 마이너)를 선택해 각각의 과에서 1개월씩 교육받도록 돼 있습니다. 결국 한국 의대생이 더 다양한 과를 경험할 수 있는 셈입니다(한국에서는 메이저 과 모두와 10개 마이너 과에서 임상실습이 진행됨). 또 다른 차이는 학생의사의 역할이었는데, 미국에서는 내과 실습 시 학생 한 명당 환자 6명을 배정받고, 그들을 진찰하고, 차트에 경과를 기록하고, 검사를 의뢰하고, 약물을 처방하고, 타과에 자문을 요청할 수 있도록 하고 있습니다. 아마도 양국이 이렇게 다른 점은 문화적 차이 때문일 것입니다.

It was also interesting to see how the differences in the insurance system allowed for a rippling effect in what diagnostic studies were ordered and what diseases were diagnosed more frequently as a result. The massive volume of patients seen each day in the clinics was impressive and overwhelming. I came away from numerous conversations about Korea’s insurance system, wondering what the manifestations of such a system in the United States would look like. Seeing firsthand such a health insurance system in practice was enlightening and I came away with a better understanding of it as a result of my time at YUMC(Yeungnam University Medical Center).

양국 보험체계도 매우 달라 한국에서는 더 많은 환자들이 정밀 검사를 받을 수 있었고, 결과적으로 질환 진단율에도 영향을 미치고 있었습니다. 매일 병원을 방문하는 너무나도 많은 환자들을 보면서 깊은 인상을 받았습니다. 한국 보험체계에 대해 대화를 나누며, 미국에서는 그러한 체계가 어떤 영향을 미칠 수 있을지 생각해보았습니다. 건강보험체계를 병원에서 직접 접하면서 많은 것을 깨우칠 수 있었습니다.

Due to the hospitality of numerous medical students, interns, residents, and attending physicians, my time at YUMC was very enriching and rewarding. I am forever indebted with gratitude to so many people. I am now in the middle of applying to Internal Medicine residencies all over the United States to begin as a first year resident in July 2010. Until then, I will be in Tampa, Florida finishing up my fourth year of medical school. I am looking forward to meeting the next group of YUMC students who will be coming to Tampa this January!
